Persons served: Families, agency personnel and professionals serving children ages birth to five with special needs.

Brief description: This council is designed to network families and professionals with special needs children. Information about events, policies, training is shared. Committees are: equipment loan, child find/transition, family liaison, public awareness and service coordination. Committees produce projects that serve families re: resource directory, equipment loan program and transition agreement.

Fee: None; parents receive a stipend to cover childcare expenses.

Qualifications: Any person with a personal or professional relationship to a young child (ren) with special needs.

Accessibility: Our meetings are held at Centerpoint Behavioral Health Center, which is wheelchair accessible.

Hours: Meetings are held the 2nd Wednesday, bi-monthly (September, November, January, March

and May with a summer break).

Brief description: Provide 16 services and do evaluation/assessments for families. If eligible, write IFSP and individual family service plan for services.

Fees: Sliding scale fee up to $100

Qualifications: 30% delay in one or more areas of communication, social, emotional sensorimotor, cognitive and adaptive skills. Medical diagnosis. Clinical opinion from professional. At risk of having a substantial developmental delay.

Insurance: Insurance billed only at parent's request.

Brief description: The Child Disability Clinic is an interdisciplinary diagnostic clinic offering a team diagnostic evaluation for children with various types of disabilities, including physical handicaps, learning problems, behavior disorders, neurological deficits and medical conditions. The process includes a comprehensive evaluation followed by medical therapy and educational recommendations.

Fees: Varies

Qualifications: Team members include a developmental pediatrician, pediatric neurologist, child psychologist, nurse, speech language therapist, occupational therapist, audiologist, social worker and educational specialist.

Insurance: Based on the insurance carrier.

Persons served: Persons birth -- 21 years of age with eligible medical conditions.

Brief description: DSCC offers care coordination, benefits management, and financial assistance for the diagnosis and treatment of children from birth to age 21 with eligible special health care needs. Special health care needs include chronic disabling conditions such as heart defects, orthopedic conditions, cerebral palsy, hearing impairment, neurological disorders, urinary system problems, some eye impairments, certain inborn errors of metabolism, hemophilia, cystic fibrosis and some craniofacial conditions such as cleft lip/palate. Financial assistance is based on parental income and available private insurance must be utilized prior to expenditure of DSCC funds. Care coordination and benefits management is provided to eligible children regardless of financial eligibility.

Brief description: Swann Special Care center is a 104-bed skilled pediatric facility for children (and adults) with severe and profound disabilities. Programming includes a private, accredited school and an adult day training program. Services provided include medical nursing, physical and occupational therapy, speech/language, nutrition and behavioral and psychological therapy.
