Persons served: Individuals 13 years and older who have experienced neurological impairments which impair their functional independence.

Brief description: Coordinated outpatient therapy approach designed to offer community focused rehabilitation services directed towards improving quality of life through re-integration into the home, community, job site and/or school.

Fees: Vary depending on therapy costs

Qualifications: Clients may be referred to the program by their physician. Formal eligibility and a treatment plan, if appropriate, are determined after an initial evaluation with therapy staff.

Insurance: May be covered by private insurance, worker's compensation or the Office of Rehabilitation Services.

Accessibility: Wheel chair accessible

Hours: Monday-Friday, 8 a.m.-5 p.m.

Brief description: Aid to the Aged, Blind or Disabled (AABD) - This program is for people who are elderly, blind or have a disability and need money and medical care to help take care of themselves. Benefits may include financial assistance, food stamps, medical assistance, dietary assistance and funeral assistance.

Temporary Assistance for Needy Families (TANF) - This program is for families with children who need temporary financial and medical assistance.

Hours: Monday-Friday, 7:30 a.m.-7 p.m. (except state holidays)

Persons served: Persons with disabilities who have the ability to work full-time.

Brief description: Serving people 15-65 years of age to help them find full-time employment. Home services programs helps people live at home instead of a nursing home. Must be a nursing home candidate and Medicaid eligible.

Fees: None. Those with high incomes may need to pay a part of their own rehabilitation costs.

Qualifications: Must be a Vermilion or Iroquois County resident to use this office, have severe disability and have desire and ability to return to work.
